JOB SUMMARY:
Provide case management activities for clients and their families. Work collaboratively with Child & Family Specialist to establish services for clients. Convenes child and family meetings to establish plan of care; continually assess and evaluate and modify plan as necessary. Promote family decision making and consensus building. Provide outreach and emergency services and counseling in crisis. Maintain consistent and regular attendance. Maintain confidentiality as required by licensing, HIPPA, and other regulatory agencies.
QUALIFICATIONS:
Masters Degree in Social Work, Counseling Psychology or related field. Previous Wraparound Program experience preferred. Previous case management experience required. Must posses and maintain a valid CA drivers’ license, automotive liability insurance, and a driving record acceptable to agency’s insurance carriers. Good oral, written and organizational skills. Physical ability to perform ProAct, administers first aid/CPR, and operate a motor vehicle.
